New Patients

Welcome to our practice! To make the check-in process easier, please download, print, and fill out our New Patient Registration Forms and bring them to your first appointment. Links to electronic versions of these forms can be filled out directly from your phone. Those links will be sent via text to a phone number provided while scheduling. Your forms will be sent directly to us once submitted. You should receive this text one or two days prior to your scheduled visit with us.

Payment and Insurance Policy

We accept most PPO plans. However, be sure to call our office prior to your appointment and we will provide you with a complimentary benefits check. If we do accept your insurance, any co­ payments and/or deductibles will be due at the time of service. We do our best to provide an accurate out-of-pocket cost for every appointment, however this is only an estimate. Payments can be made using Visa, Mastercard, Discover and American Express. We also accept cash, personal checks and CareCredit.
